Marching Band 2008 Information

Once you print the forms, SIGN THE FORMS and return the forms to the band office or to your middle school band director. You may complete the perscription drug forms at a later time
but you must bring the other forms along with the initial deposit of $125.00 before May 16th. (Make checks payable to “Fairmont Band Boosters”
If you encounter problems with the online forms, contact your child’s band director and they will send
copies of the forms home to you to fill out.
The comprehensive fee for marching band is $415. There is a $10 discount for paying the entire balance before May 16th.
There are a limited amount of scholarships and grants available to those families who can provide proof of financial need.
Student Account Balances: Families may only use their student account to the pay the first payment if they have at least $405 in their account.
If you do, please indicate that on the registration form and we will deduct the full amount from your account.
If you do not know your balance, contact the band office and they will send it to you.
(2008 Pizza and Sub sale balances will not be available until after the initial deposit is due)
note that you can type the information directly in to these forms and then print them.
